There are two types of disability pension: "Basic Disability Pension" and "Employees' Disability Pension." If you were enrolled in the National Pension Plan when you first saw a doctor for illness or injury, you can claim "Basic Disability Pension." If you were enrolled in the Employees' Pension Plan, you can claim "Employees' Disability Pension."
In addition, if you are left with a disability that is less severe than that which qualifies for disability employees' pension insurance, there is a system in place that allows you to receive disability allowance (lump sum).
Also ,
In order to receive disability pension, there are certain conditions, such as pension payment status.
Those receiving the Basic Disability Pension or Employees' Disability Pension (limited to those with a disability level of 1 or 2) are exempt from paying national pension insurance premiums ( statutory exemption system for national pension insurance premiums ). Three things you need to do to receive benefits

01
Confirmation of first consultation date
-
The first visit date for the illness or injury that caused the disability must be between the following dates:
・Period of enrollment in the National Pension System before the age of 20 or period between the ages of 60 and 65 while living in Japan and not enrolled in the pension system
Date of first medical examination This refers to the date on which a patient first received medical treatment from a doctor for the illness or injury that caused the disability or death. If a patient changes doctors for the same illness or injury, the date on which the patient first received medical treatment from a doctor is considered the date of first medical examination.
02
Have you paid your insurance premiums properly?

On the day before the first medical examination, the period of insurance coverage up to the month before the month in which the first medical examination occurs must be two-thirds or more, including the period in which you have paid National Pension insurance premiums (including the period insured under Employees' Pension Insurance and the period in which you were a member of a mutual aid association) and the period in which you have been exempt from insurance premiums.
However, if the first consultation date falls before April 1, 2026, and you are under 65 years old on the first consultation date, you are only required to have no unpaid insurance premiums in the past year up to the month before the month in which the first consultation date falls on the day before the first consultation date.
In addition, if the first medical examination date occurs during the period before the age of 20 when you were not enrolled in the pension system, there is no payment requirement.

03
Does it qualify for a disability rating?
The state of the disability must be Grade 1 or Grade 2 as defined in the disability rating table on the date of disability certification (or on the date of turning 20, if the person turns 20 after the date of disability certification).
Disability Certification Date This refers to the date on which the condition of the disability is determined. It is the date one year and six months after the first medical examination for the illness or injury that caused the disability, or the date on which the illness or injury has been cured (symptoms have stabilized) within one year and six months. If any of the following dates occur within one year and six months from the first medical examination, that date will be considered the "disability certification date." If you are undergoing dialysis, the date three months after the first dialysis session. If you have had an artificial femoral head or artificial joint inserted or replaced, the date of insertion. If you have had a cardiac pacemaker, implantable cardioverter defibrillator (ICD), or artificial valve inserted, the date of insertion. If you have had a colostomy or urinary diversion, the date six months after the date of insertion or surgery. If you have had a neobladder inserted, the date of insertion. For limb disabilities due to amputation or severance, the date of amputation or severance is the general rule (for disability benefits, the date the wound heals). For total laryngectomy, the date of severance. For home oxygen therapy, the date home oxygen therapy began.
